<!-- wp:paragraph --> <p>Millions of Americans <a href="https://www.kff.org/public-opinion/kff-health-tracking-poll-prescription-drug-costs-views-on-trump-administration-actions-and-glp-1-use/">take GLP-1 drugs</a> like Ozempic or Wegovy. Millions more<a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC6070393/"> take stimulant medications such as Adderall</a>. As those two groups increasingly overlap, many people are asking the same question: Is it safe to take them together?</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>For most people, taking them together isn't off-limits, but there are potential risks that shouldn't be overlooked.</p> <!-- /wp:paragraph --><!-- wp:heading --> <h2 class="wp-block-heading">How GLP-1s Work</h2> <!-- /wp:heading --><!-- wp:paragraph --> <p>There are <a href="https://www.endocrinologyadvisor.com/features/mechanism-of-action-glp-1-agonist/">six different kinds</a> of GLP-1s that have been approved by the FDA as active ingredients for diabetes and/or weight control – dulaglutide, exenatide, liraglutide, lixisenatide, semaglutide, and tirzepatide. </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>“GLP receptor agonists are essentially mimicking the action of a natural hormone that our body makes called glucagon-like peptide-1,” explains <a href="https://medshadow.org/author/sararogers/">Sara Rogers, PharmD,</a> president of the American Society of Pharmacovigilance, a nonprofit network of healthcare professionals dedicated to preventing adverse medication events, and a member of MedShadow’s Health and Medical Advisory Panel. “The purpose of this hormone is to help our body regulate blood sugar, appetite, and digestion.“ </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>GLP-1s interact with glucagon-like peptide-1 receptors in your <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC8348147/">pancreas</a> to regulate blood sugar and receptors in your brain to reduce appetite and create a feeling of satiation. They also slow down gastric emptying, meaning that food stays in your stomach longer. </p> <!-- /wp:paragraph --><!-- wp:heading --> <h2 class="wp-block-heading">Could GLP-1s Delay How Adderall Works?</h2> <!-- /wp:heading --><!-- wp:paragraph --> <p>The fact that GLP-1s cause slower gastric emptying has led some people to speculate about whether this could affect how quickly or how well Adderall will work. That’s because drugs tend not to be absorbed well by the stomach — it’s when they move on to the <a href="https://pubmed.ncbi.nlm.nih.gov/797497/">small intestine</a> that they’re quickly absorbed. So even if GLP-1s don’t directly interact with Adderall, delayed stomach emptying could theoretically change how quickly the medication would take effect.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>The experts MedShadow spoke with were unaware of any studies specifically examining Adderall absorption, but a <a href="https://link.springer.com/article/10.1007/s40264-023-01392-3">recent review </a>of GLP-1s taken with other oral medications found that there could be delays, though none were considered clinically significant. Nonetheless, there is at least one (unverified) report on <a href="https://www.reddit.com/r/adhdwomen/comments/1kvixb6/adhd_and_glp1_interaction_doctor_doesnt_have/">Reddit</a> describing an Adderall dose that took up to eight hours to take effect, while other commenters in the same thread said they had not noticed any change.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>How exactly absorption issues might manifest depends, in part, on which of Adderall’s many formulations you take, points out <a href="https://dellmed.utexas.edu/directory/michelle-magid">Michelle Magid, M.D</a>., associate professor for UT Dell Medical School and Texas A&M Health Science Center, and president of Austin PsychCare in Austin, TX. </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>“The Adderall immediate release (IR) usually kicks in pretty soon — people can feel the differences in 30 minutes, with the peak effect in three hours,” she explains. With a GLP-1? “What you might see is people not [experiencing] that effect until an hour later.” </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>For people taking Adderall to manage ADHD, even small changes in when the medication kicks in can matter, but being aware of the potential delays in uptake time may help people work with their doctors if they start taking both drugs. While a different time-release formulation might work better, generally, “the overall exposure or the amount of drug that gets absorbed in the blood would probably be similar, even with a delayed gastric emptying,” according to Dr. Rogers. </p> <!-- /wp:paragraph --><!-- wp:heading --> <h2 class="wp-block-heading">GLP-1s and Adderall Can Both Raise Heart Rate</h2> <!-- /wp:heading --><!-- wp:paragraph --> <p>The more potentially dangerous concerns about taking both medications may not come from the way they interact, but from the way their effects can unexpectedly reinforce each other. There are two major areas to be aware of here: Heart rate and weight loss. </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Both Adderall and GLP-1s have been associated with modest increases in heart rate. For Adderall, that’s not surprising — it’s a mix of amphetamine salts — and it can raise blood pressure too. </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>“[Adderall] mimics a fight-or-flight response in the system,” says Dr. Magid. “The good part about that is that it will improve attention, focus… and impulse control.” The medication increases <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC8063758/">dopamine and norepinephrine activity</a>, brain chemicals that help people stay alert, motivated, and organized. In people with ADHD, boosting those signals can make it easier to concentrate, prioritize tasks, and resist distractions. But because <a href="https://www.ncbi.nlm.nih.gov/books/NBK507716/">norepinephrine</a> also prepares the body for action, it can increase heart rate and blood pressure.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Indeed, a <a href="https://pubmed.ncbi.nlm.nih.gov/22796229/">meta-analysis</a> of studies looking at adults taking central nervous system stimulants for ADHD found an increase of 5.7 beats per minute in heart rate and 2 mm Hg in systolic blood pressure. And a <a href="https://pubmed.ncbi.nlm.nih.gov/41770187/">recent, smaller study</a> published in <em>Mayo Clinic Proceedings</em> showed that in healthy young adults who had not taken Adderall before, a single dose (25 mg) of Adderall raised heart rate an average of 10 beats per minute and systolic blood pressure by 10 mm Hg (measured three hours after dosing). </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Perhaps more surprisingly, GLP-1s can <em>also</em> raise heart rate, according to the experts MedShadow spoke with. One small study from 2017 found that taking a GLP-1 for 12 weeks increased heart rate in patients with type 2 diabetes by an average of <a href="https://pubmed.ncbi.nlm.nih.gov/27777261/">6.6 beats per minute</a>, but lowered systolic blood pressure by 12.6 mm Hg. The mechanism behind this isn’t fully understood, but according to Dr. Rogers, researchers think GLP-1s may affect the heart's pacemaker cells, which regulate its contractions.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Although both medications can raise heart rate, Dr. Magid says the combined effect is usually moderate enough that it is not the most significant health concern for most people.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>She notes that <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC7851748/">stimulants</a> are often prescribed to younger people, who tend to have better heart health. “I’m less worried about a heart attack in a college kid than I am in a 70-year-old,” she says. But for anyone taking this combination, she adds, it’s still wise to monitor blood pressure.</p> <!-- /wp:paragraph --><!-- wp:heading --> <h2 class="wp-block-heading">GLP-1s and Adderall Can Suppress Appetite</h2> <!-- /wp:heading --><!-- wp:paragraph --> <p>A more worrisome problem may be appetite suppression. Together, the drugs could make some patients less likely to eat or drink enough, potentially leading to rapid or excessive weight loss. This may seem ironic, since one reason many take GLP-1s is precisely to lose weight and Adderall has long been known to <a href="https://www.sciencedirect.com/science/article/abs/pii/S0195666313000287">suppress appetite</a>. But the danger is real. </p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>“If you become malnourished, we're talking about cardiovascular risk, we're talking about endocrine risks, infertility, amenorrhea,” says Dr. Magid. “We're talking about neuropsychiatric risks, depression, cognitive impairment, and wound, healing, and skin risks.”</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Anecdotal evidence from <a href="https://www.reddit.com/r/adhdwomen/comments/1kvixb6/adhd_and_glp1_interaction_doctor_doesnt_have/">Reddit users</a> on the platform’s r/ADHDwomen page suggests that this isn’t just a theoretical concern, with some reporting that they have no appetite and one claiming to have been “basically anorexic” on the combination — though others say their ADHD symptoms have improved with a GLP-1 because they’re no longer plagued by “food noise.”</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Why can the effects be so exacerbated? In part because the appetite-suppressing mechanisms are different. While GLP-1s suppress appetite and create a feeling of satiation, Adderall essentially works by making focus feel more rewarding than other activities, like eating, explains Dr. Magid.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>Another concern for doctors is the possibility that people with disordered eating might use the drug combo on purpose to suppress appetite and help them lose weight quickly. Virtual visits, in which patients are not required to attend in-person weigh-ins, can make it easier for patients to misrepresent their weight to healthcare providers.</p> <!-- /wp:paragraph --><!-- wp:heading --> <h2 class="wp-block-heading">How to Take GLP-1s and Adderall Safely</h2> <!-- /wp:heading --><!-- wp:paragraph --> <p>If you’re taking multiple medications, it’s always important to make sure one medical professional reviews all your prescriptions. “A GLP-1 might be prescribed by an endocrinologist, and the Adderall might be prescribed by a psychiatrist,” says Dr. Rogers. She notes that, most often, it falls to the primary care provider to optimize and manage the various medications.</p> <!-- /wp:paragraph --><!-- wp:paragraph --> <p>While the evidence available today suggests that many people can safely take GLP-1 medications and Adderall together under a healthcare provider's supervision, everyone’s medical histories, metabolisms, and medication responses differ. Anecdotal reports can highlight questions research hasn’t yet answered, but they can't predict how you personally will respond. If you notice unexpected changes after starting either medication — such as a delayed effect from your Adderall, a racing heart, or unintended weight loss — don't ignore them. Keeping track of those changes and discussing them with your healthcare provider can help determine whether your treatment needs to be adjusted.</p> <!-- /wp:paragraph -->
